Forms Required for Boarding
Attention All Boarding Clients:
U Dirty Dawg can no longer accept your dog for boarding without current pet and owner information filled our on our website for new and existing clients and proof of current vaccine history before drop off.
This information must be provided, copied and/or emailed PRIOR to drop off, we cannot copy it for you. We will also no longer be able to call your dog’s veterinary clinic to verify if your dog’s vaccines are current unless they are a patient of Center Grove Animal Clinic.
Since we do however require Bordetella and a Fecal Exam every 6-months, we can still do that at Center Grove Animal Clinic during boarding. However, all other annual vaccines will need to be scheduled prior to boarding even if your dog is a patient at Center Grove Animal Clinic. This will be effective immediately.
If you're visiting U Dirty Dawg for the first time, you'll need to fill out our pet service forms before accommodating your pet at our boarding facility.
